Insurance

We strive to provide the highest quality of healthcare to all our patients. In order to facilitate the insurance process, we will bill and collect from the designated primary insurance carrier. Although we are not required to submit secondary claims for patients, we will do it as a courtesy. Each patient presenting with an insurance card will be confirmed for eligibility at the time of service. Patient presenting with ineligible insurance will be required to pay for the office visit at the time of service and submit a reimbursement request to their insurance company.

We accept cash, check and credit card payments. We belong to most major health care plans, Advocate Health Partners Plans, and Little Company of Mary Health Plans. We ask that patients verify our participation with their plan prior to their appointment due to the rapids changes in the health insurance arena.

Our Physicians currently are active on staff at Advocate Christ Medical Center in Oak Lawn, IL and Little Company of Mary Hospital in Evergreen Park, IL.
